How can I get a copy of my medical records?
1. During the consultation by appointment, you can directly ask the doctor or medical personnel for the medical records.
2. If you have not made an appointment, you can download and fill out the Application for Duplicate Medical Records from Taipei City Hospital website and go directly to the Certificates counter in the Admission Center to request the Certificate.
