What are the required identifications for the medical certificate?
1. Please register for an appointment followed by requesting your doctor for the certificate during your consultation.
2. The applicant should be the patient him/herself with the National Identification (ID) Card & Care Card.
3. If not the patient him/herself, only family members directly related to the patient lineal relative provided with the power of attorney would be able to apply for the request.
