1. All proof of emergency medical service for emergency rescue cases within the last 7 years can be requested by calling 119 (free service). 2. May apply personally at the Fire Department, by fax (2345-6386) or by mail (No.1, Songren Road, Xinyi District, Taipei City 11073). 3. On the Fire Department’s website (http://www.tfd.gov.tw/)(Chinese), click on “Forms Download” and select “EMS proof.”
